FormsAuthorization to Release Information -
This form gives your authorization to MU to discuss your loan account with a third party (parent, spouse, etc.) or to discuss via a specific e-mail account.
Easy Pay Automatic Withdrawal -
This authorization form allows MU to automatically debit your checking or savings account for your monthly payments. Please see our Easy Pay Automatic Withdrawal webpage for complete info.
Economic Hardship Deferment Form -
If you feel you qualify for Economic Hardship and you have a Perkins loan, fill out and return this form. Your financial situation must meet Department of Education criteria.
Personal Data Sheet -
To complete Exit Counseling on loans collected by MU, this form must be completed and returned along with your Rights and Resonsibilities sheet which would have been mailed to you.
Statement of Financial Condition Form -
This form must be completed in order to request a temporary repayment arrangement.
Student Deferment Form -
If you are enrolled at least half-time as a degree-seeking student at an eligible school and you have a Perkins, NDSL, and/or University Long Term Loans, use this form to request Student Deferment. You must fill out Part I completely. Your school must complete Part II to certify your enrollment.
